TDLR Supplies all of the required forms as well as the complete Texas Accessibility Standards on their website. Click here for a pdf version.
We will require a full set of plans for each discipline. This ensures a thorough and complete plan review. We can accept full size or 1/2 size sets by mail or you can e-mail PDFs or supply FTP instructions.
All forms should be filled out electronically using Adobe Reader. You can download it for free by clicking the link below.

Project Order Form
For this to work correctly, you will need to right click and save as. Then open the file on you local drive.
Just fill it out and click the “Email to TAS” icon. We will use this information to populate the fields on all the required forms for your convenience. You don’t need to worry about any of the forms below for now.

Project Registration
This form registers the job with TDLR. This is also how you obtain your EAB Project Number which is required in many jurisdictions to obtain a permit. There is a $175 State Filing Fee. We can file the form for you once it is filled out and signed by the owner. Per state rules, we must collect our fees up front. The total for most jobs comes to $925 for the plan review, inspection, and state filing fee. This is payable by check to “Texas Accessibility Solutions”.
Proof of Submission
This form must be filled out within 20 days of the issuance of the construction documents. It must be signed by the Design Professional.
Owner/Agent Designation
This form must be filled out by the owner to allow someone other than the owner to sign the request for inspection.
Request for Inspection
Pretty self explanatory. Must be signed by the owner, or the designated agent on the previous form.
